When each answer is the safer default
Choose patch when the path is clear
Patch is the safer default when the affected asset is confirmed, the vulnerable version is present, a fixed version exists, the owner is known, rollback is understood, and the exposure or exploitation signal justifies change risk.
Choose mitigation when risk cannot wait for patching
Mitigation is the safer default when no fix exists, the patch is unsafe, the window is delayed, business impact needs approval, or exposure must be reduced while validation continues.
Choose monitoring when pressure is not proof
Monitoring is the safer default when the product may not be present, affected versions are unconfirmed, exposure is weak, the vulnerable feature is disabled, or public chatter has not become environment-relevant risk.
Escalate when the blocker is a decision
If the team cannot choose because ownership is unclear, downtime needs approval, customer impact is likely, vendor guidance conflicts, or incident-response criteria may apply, escalate the decision instead of letting the item drift.

Questions that prevent bad assignments
Is the affected version actually present?
If not known, investigate before assigning patch work. A scanner match or product-family advisory is not enough by itself.
Can an attacker reach the vulnerable path?
Internet-facing, unauthenticated, management-plane, identity, and business-critical paths change the urgency of the lane.
Does a safe fixed version exist?
If the fix exists but cannot be deployed safely, mitigation and escalation may be better first moves than forced patching.
What evidence would change the lane?
Define the trigger: KEV inclusion, confirmed exposure, vendor update, failed validation, detected abuse, or owner approval.

Common patterns and safer defaults
KEV plus internet-facing plus fixed version
Default: patch. Add rollback, owner, and post-patch proof. Mitigate only if the window or fix is unsafe.
No patch for exposed service
Default: mitigate. Apply vendor workaround, restrict access, increase detection, and set a review date.
Critical score but feature disabled
Default: monitor. Save configuration evidence and revisit if the feature changes or vendor guidance narrows.
Public PoC but inventory uncertain
Default: investigate and monitor. Validate product, version, reachability, and telemetry before creating urgent patch work.
Patch available but customer outage likely
Default: mitigate and escalate. Restrict exposure while the owner approves a safe change plan.
Vendor advisory conflicts with scanner output
Default: investigate. Compare vendor affected ranges, CPE/product names, installed versions, and source confidence.
